Create a visio diagram from an existing image

I have an image (attached) which contains a diagram which I need to be recreated for use with Microsoft Visio . The result must be a visio file that can be viewed and edited using Vision 2013.
Additionally I would also like a frame template to be added to allow title, author date, revision etc to be visible in the bottom right of the screen if possible.
