Create Spreadsheet of Contact Info from Scanned Document - URGENT IMMEDIATE RESPONSE REQUESTED

Seeking freelancer to create a contact information spreadsheet from a scanned document. Document contains approximately 170 companies and up to 300 names. Current information to be put into spreadsheet includes company name, contact names, mailing addresses and phone number. Freelancer will also conduct a basic internet search/make phone calls to find email addresses for contacts and add to the spreadsheet. This is a highly detailed job. Speed and accuracy are paramount.
