Data appending for our excel spreadsheet of leads

We are a UK business with 2 worksheets,

1. Leads worksheet, which have most dialling codes set out where (01234) becomes (+44 1234) for Skype click to call. Our data also has a lot of missing "City" column information.

2. UK Area Codes Worksheet, Where the dialling codes are currently (01234) format with the column "City" filled in.

We need the UK Area Codes to be in the Skype friendly format (+44 1234) and the "City" to be appended in our lead worksheet so we are able to filter by city within our CRM.

I have supplied the full worksheet for UK Area Codes and a Sample of our Leads below for your information.

Within the Sample Data you will see where we need the City appended.

If anyone is able to perform this task then please let me know ASAP as I have spent hours on it!! I have been given the following formula to fix the issue but am unable to implement it on our data spreadsheet:
