I need to print my invoices on the Google Spreadsheets. Each print to be filled through the form created within the spreadsheet. The design of the A4 invoice is attached below ([login to view URL]), please check it.
The job includes:
* Creating a form with the required fields.
* Adjusting the form fields to be printed on attached A4 invoice.
* Each form should be named manually by the inputter.
* Please be informed that, the attached image of A4 invoice supplied ([login to view URL]), is to be used for adjusting the print area only. It should not be used as background or in any way. The actual invoice paper is already preprinted and the information inputted through the form will be printed over it. In other words, I already have the attached design printed over on the paper. So we only need to print the filled forms over it.
* There are 12 fields in total. Field names will be the same as the design in a4-fatura.jpg.
* Just for information only, please check the meaning of the field names via TRANSLATION OF THE [login to view URL] attached.

I have created a similar tool for somebody on Google spreadsheet. That does all the calculation for total work and effort for writers. Then it generates a report. I can match your requirement. The reason I kept three days as day 1 will go in designing, day 2 in coding. Day 3 is important as I just don't want to develop and hand over, I want to ensure that it is working for you and if you want some changes in final product.
