Build Intelligent Forms

We need approximately 20 web forms created. These forms will be linked from a common database (MSSQL). There will be a profile required that will include basic contact information (name, address, city, state, zip, etc.) from this profile the used can then select 1 of these 20 forms. When they submit the form, a copy will be sent via email to the administrator, and a copy written to the database and stored. The user then can either re-access the form or has the option to delete the form from their profile page when they log back in. These forms are basic insurance application pages. It would be nice if they were broken up into several pages with a progress indicator showing how many pages are left. We also have information from an online dictionary that needs to be added to the form fields, so when a user mouse overs a word, the definition appears (we will provide this content). This system needs to support multiple insurance organizations under one commond database. Meaning, there needs to be an administrative area to add an agency. Hierarchy: ADMIN |_ AGENCY | _ CUSTOMER This is the start of this project, and the winner will get additional development work. Your code needs to be clean and easy to add on to as we have additional plans for this system; and we will want you to do the work.
