write a whitepaper geared toward the Managed Care Organization audience. The white paper would be a high-level thought leadership piece 4-5 pages long with compelling content from research gathered from Gartner, Forrester, Medical Forums, Technology Forums, etc. We are looking to have this completed asap.
Here is a general outline of what we would want the piece to over….
5 Key Considerations for Managed Care Organizations to Factor When Building a Technology Roadmap (open to updating this to be more appealing)
1. Learning more from the data you’re receiving from outside sources - claims, census, etc…
a. Mine the data and flag health conditions
b. Determine what needs to be investigated and what needs to be managed
2. Persuasive, consistent telephonic outreach
a. Create a culture of informed, persistent outreach.
b. Never miss a beat – use queues and workflows to prevent orphaned, dormant contacts and to transfer call loads to when resources are sick or out of the office
c. Become a metrics powerhouse – capture critical data and use it to drive improved performance in the future.
3. Effective, accurate scheduling of field resources – the cost of putting the wrong person with a client is high
a. Resource proximity – home base and prior and subsequent visits
b. Effective communication – language barriers
c. Business rules – plan requirements, employee vs 1099,
4. Smart, Actionable Assessments
a. Get off paper – push and pull rich data directly into your system
b. Remove the waste - Make the process faster and more efficient for patients and care providers (talk about CCS time savings)
c. Get better answers – Ask the right question at the right times and learn more about your patients. Talk about CCS 600% and 200% improvement numbers
d. Drive to improve outcomes – leverage what you learn to make recommendation, in real time when the patient is engaged and your resource is in place
5. Compelling, personalized care management
a. Utilize the information you’re learning from home visits and health assessments including critical environmental health data
o Flag problems and work with patients to set personalized goals and intervention strategies that they are vested in accomplishing
o Track progress over time, correcting for missteps and involving patients and family members to maintain improved outcomes
