We are wanting an app designed for our business. It should be fairly straight forward. I have attached a couple of pics but would like the person we choose to make it look professional and sleek. Below I have outlined our requirements. Please feel free to ask questions. 1. Ask a question button > When a user clicks on ‘Ask a question button’ the user enters their workplace password > Which then populates a questionnaire form (details of form below) Ask a question Form Name Email Phone Question 2. My Workplace button > When a user clicks on ‘My Workplace button ‘the user enters their workplace password > Workplace Info Page • List of Monthly Health Articles > Downloadable PDFs • Health Links > page with links and contact numbers • Ask a Question > Ask a question form (1) • Submit a dream ticket > link to survey gizmo form 3. Facebook > Facebook feed 4. Contact page (website, phone numbers, location etc)
