Acrobat Online Form with Calcs

Utilizing a draft word document, design a 4 page fillable form in acrobat designer as it involves several scripts. These scripts involve date calculations related to pregnancy or body mass index. Also, several fields have drop down menu, and if certain item is picked, another area on form will be flagged (ie if Yes, then another area flagged or highlighted). Explain and implement so that the client can complete form on my web server, print it, or submit it as an e-mail attachment using my servers e-mail rather then relying on the individual's own e-mail program. I'll provide all the calculations. Essentially, transcribe draft doc into designer, implement scripts, and upload feature. I can send a draft form of calcs required to interested individuals.
