コンテスト概要

I would like to work with a graphic designer to create a 2-page order form for an educational business. We provide competitions that take place over set dates. As such, the two parts are (1) a competition calendar with key date windows highlighted and (2) an order form/input page where customers can input their information and select which products they'd like to purchase

The MAIN thing I am looking for is a PDF order form that is well-designed and utilizes graphic(s) to show our competition dates and purchasing information in a user friendly manner. The form (below) is an added bonus, not the main deliverable.

Please provide any relevant details about your portfolio to aid in the decision-making process, and let me know what additional information would be needed to complete the project. I have attached the necessary copy/words in the .docx file, and the required information can be found in the link below or in the attached PDF. I have also attached out logo as a png.

BONUS point: Translate the key inputs for the document into a Google Form or Jotform to enable customers to submit information online
